Job Description:
JOB SUMMARY
The primary responsibility of the Physical Therapist (PT) is to
plan, delegate and provide safe and competent physical therapy
interventions as ordered by physician.
ESSENTIAL FUNCTIONS
Provide physical therapy assessments, as well as develop and
implement individualized treatment plans to increase client's
independence and functioning
Supervise Physical Therapy Assistants, provisional licensees,
students and support personnel in accordance with state licensure
requirements and professional standards
Identify need to make referrals to other disciplines to address
comprehensive care needs of the client
Demonstrate competent technical skills according to current
Physical Therapy standards of practice
Communicate changes in resident's condition
Participate in discharge and community mobility planning
Prescribe necessary durable medical equipment to facilitate
independence
Provide accurate and timely billing records and supervise
supporting clinical documentation
Respond to family requests and concerns timely
Assure resident abuse or neglect does not occur
Observe confidentiality and safeguards for all patient
information
Participate in collaborative care conferences
Coordinate and provide training with Restorative Nursing and other
Care staff
Provide education and training to patient, caregiver, and community
partners
Assist with training and education of facility and professional
partners
Ensure compliance with mandatory education, training and competency
programs
Performs other duties as assigned
JOB QUALIFICATIONS
Education
Graduate of an accredited physical therapy school.
License
Current Wyoming Physical Therapy License
Certifications required
See Cardiopulmonary Resuscitation Certification Policy and
Certifications/Education Requirements Policy.
Experience
Experience in providing appropriate care in post-acute or long-term
care environment preferred
Knowledge, Skills, and Abilities:
The ability to maintain continuing education requirements and all
certifications and licenses as required to maintain professional
license.
Licensed professionals must strictly adhere to the ethical code of
their respective discipline.
Must use good judgment in interpreting patient referrals and
performing appropriate physical therapy procedures.
Ability to develop positive interaction with patients, patient
families, physicians, and staff in order to effectively care for
the patients.
Must be able to assess a patient's condition, formulate a plan of
care, select appropriate interventions, and evaluate patient
response to care/treatment and to explain/teach patients about
their condition/recovery.
Ability to perform multiple tasks simultaneously with frequent
interruptions.
Analytical and reasoning skills with a high level of mental
concentration and memory.
Ability to speak clearly to communicate and interact positively
with patients, patient families, physicians, and staff in order to
effectively care for the patients.
Computer Knowledge including the ability to enter material into
electronic record. Ability to operate fax, computer, and copy
machine to perform the duties of the position.
The ability to demonstrate knowledge of developmental stages and
apply developmental theories/concepts when planning and
implementing care for the appropriate age of the patient. Ability
to maintain the appropriate level of knowledge and skills.
