JOB SUMMARY

The primary responsibility of the Physical Therapist (PT) is to plan, delegate and provide safe and competent physical therapy interventions as ordered by physician.

ESSENTIAL FUNCTIONS

• Provide physical therapy assessments, as well as develop and implement individualized treatment plans to increase client's independence and functioning - - - -
• Supervise Physical Therapy Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards - - - -
• Identify need to make referrals to other disciplines to address comprehensive care needs of the client - - - -
• Demonstrate competent technical skills according to current Physical Therapy standards of practice - - - -
• Communicate changes in resident's condition - - - -
• Participate in discharge and community mobility planning - - - -
• Prescribe necessary durable medical equipment to facilitate independence - - - -
• Provide accurate and timely billing records and supervise supporting clinical documentation - - - -
• Respond to family requests and concerns timely - - - -
• Assure resident abuse or neglect does not occur - - - -
• Observe confidentiality and safeguards for all patient information - - - -
• Participate in collaborative care conferences - - - -
• Coordinate and provide training with Restorative Nursing and other Care staff - - - -
• Provide education and training to patient, caregiver, and community partners - - - -
• Assist with training and education of facility and professional partners - - - -
• Ensure compliance with mandatory education, training and competency programs - - - -

            • The ability to maintain continuing education requirements and all certifications and licenses as required to maintain professional license. - - - -
            • Licensed professionals must strictly adhere to the ethical code of their respective discipline. - - - -
            • Must use good judgment in interpreting patient referrals and performing appropriate physical therapy procedures. - - - -
            • Ability to develop positive interaction with patients, patient families, physicians, and staff in order to effectively care for the patients. - - - -
            • Must be able to assess a patient's condition, formulate a plan of care, select appropriate interventions, and evaluate patient response to care/treatment and to explain/teach patients about their condition/recovery. - - - -
            • Ability to perform multiple tasks simultaneously with frequent interruptions. - - - -
            • Analytical and reasoning skills with a high level of mental concentration and memory. - - - -
            • Ability to speak clearly to communicate and interact positively with patients, patient families, physicians, and staff in order to effectively care for the patients. - - - -
            • Computer Knowledge including the ability to enter material into electronic record. Ability to operate fax, computer, and copy machine to perform the duties of the position. - - - -
            • The ability to demonstrate knowledge of developmental stages and apply developmental theories/concepts when planning and implementing care for the appropriate age of the patient.
